Crawford County Health DepartmentThe mission of Crawford County Health Department (CCHD) is to prevent disease and to assist in the maintenance of optimum health for the citizens of Crawford County through the assessment of needs, development of policy, provision of education and the assurance of services.

Some way, every day, the Health Department has a role in your daily activities. We are guardian of your health in relation to community activities. Each individual or family can maintain their own safe environment, but, when many individuals and families come together to form a community, public health as a whole must be protected. We appreciate the trust you place in us in our efforts to accomplish this.

Physicals & Immunizations Schedule

Crawford County Health Department is scheduling appointments for school physicals and immunizations. Physicals and immunizations are by appointment only and there are a limited number of appointments available. Nurse practitioners Lynn Phillippe and Debra Waggle will be conducting the physicals.

Physical information

Appointments with Lynn Phillippe, FNP can be made for July 1 from 3:00-7:00 PM.

Appointments with Debra Waggle, FNP can be made for July 30 and August 13 from 3:00-7:00 PM.

Fees for physical services are $45.00 for a physical, $5.00 for hemoglobin, and $5.00 for urinalysis.

Immunization information

Immunizations are generally every first and third Monday evenings of the month from 4:00-8:00 PM and every second and fourth Thursday monring of the month from 8:30-11:00 AM. Making an appointment is strongly recommended to ensure that you can get in in a timely fashion. There is a $10.00 charge per immunization.

Monday evenings 4:00-8:00 PM: July 7 & 21, August 4 &18

Thursday mornings 8:30-11:00 AM: July 10 & 24, August 14 & 28

Lead screens available for $16.00 (free if income eligible).

Weekly walk-in sessions take place every Monday morning from 8:00-11:00 AM and are on a first come, first serve basis. Please plan to come during the early summer months unless you don't mind waiting as August walk-in clinics get VERY busy.

Smoke-Free Illinois

This communication is to inform you that the Smoke-free Illinois Web site is currently active. There are variety of materials available that will be helpful to the general public. Sample signs are available at the Web site. These are samples since the rules are not finalized. As stated in the e-mail from Tom Schafer sent 12/12/07:

"The act simply says the signs must include the international no-smoking symbol. The rules had proposed additional requirements for the sign, including its size and the inclusion of the Department's toll-free Complaint Line and Web site. Without rules approval, the Department's proposal for signs is not a requirement, but something we would still recommended."
